REQUIREMENT FOR PLAYERS RESIDING  in the state of Kentucky. The KY Youth Soccer office REQUIRES Ky residents to apply for an out of state release to play in Indiana. After submitting the online application you will receive an email reply with a signed and stamped form from the KY Youth Soccer Office. (If you do not receive the form please contact their office as soon as possible.) The application for the OUT OF STATE RELEASE can be found HERE.
